Please complete these forms:


  • Release of Liability and Assumption of Risks Contract
    Complete the RLAR form, include your name, address, student ID number and sign. Original copies must be sent in or handed in at the meeting.
  • Emergency Medical and Repatriation of Remains Insurance
    EMRR insurance is required for all participants travelling with the IFS group to enable participation. Most travel insurance companies issue this insurance. Fill in the Student Information Form located below, including your EMRR provider name, your policy number and the telephone number to call from the destination (not the 800 number). Attach a copy of your policy page which states that you have EMRR insurance and the amounts. (NSU employees are covered by employer insurance, check for the policy online and include your student ID number on the form).
  • Student Information Form
    Complete the SIF form and supply a copy of your passport face page.
  • US State Department Registration
    US citizens should register their travel details with the State department to take full advantage of potential assistance while travelling.
  • Film Release Form
    Complete the film release form and hand in or email
